Recently, one of our Community Health Workers (CHW) received an update from a client who had been screened for high blood pressure. During the screening the client’s numbers presented as Stage 2 Hypertensive on the “Know Your Numbers” high blood pressure scale developed by the
American Heart Association. Our CHW encouraged the client to schedule an appointment with his primary care provider to follow up on his results. Later, as our CHW was on her lunch break, she was approached by the client and he informed her that he had seen his provider and that after incorporating healthier eating habits and eliminating smoking from his daily life, his numbers had improved.
