My husband had hip surgery. He was in the hospital and was in such pain he wasn’t able to do any therapy. So instead of coming home, he had to be transferred to a rehabilitation center for over two weeks. The doctor told me they wouldn’t discharge him until home assistance was in place. I was upset and worried about him and exhausted from the daily trips up and down the coast.

I made several calls to different local organizations but nobody seemed to be following through with what we needed. Thankfully a friend of ours works for Mendonoma Health Alliance and shared about their free Care Transitions program. After our short conversation with their Community Health Worker, it seemed like everything I had been worrying about was taken care of. Heather came to our home with loads of helpful information and assistance, including a list of local caregivers. She knew what we needed before we did! Instead of having to wait several weeks for follow-up appointments, she was able to arrange doctor’s appointments in the same week. She gave us assistance with replacing my husband’s CPAP machine and even helped us get our generator going.  

Anything we needed, Heather worked it out. We are so impressed with your organization. We have nothing but praise for MHA!
