Our Services

It is MHA’s goal to provide person-centered programs that meet people wherever they are in their healthcare journey. Our services are available to anyone living in our Service Area. We provide home visits, one-on-one assistance in reviewing instructions from healthcare providers, help with preparing the client for doctors’ appointments, health education, as well as access to resources specific to each client’s needs. Care Transitions

This is a 30-day program that helps connect people to resources in order to ensure a smooth transition back home after being released from the hospital.

Care Coordination

This program provides assistance and support for a wide variety of needs, such as accessing fresh food and health education, assisting clients with paperwork, working with clients to achieve health goals, etc. This program is available to clients for as long as it takes to meet their needs.

Educational Workshops

MHA works with professionals from traditional and non-traditional health care to provide health education. Workshops are scheduled at least once per quarter and rotated throughout our Service Area. All workshops are free and available to all ages.

Healthy Living Course

This is a free class for any person who is living with a health condition that they need help managing. The class teaches participants skills to create plans and achieve short-term and long-term health goals. This is a six-week course that is scheduled weekly for two-and-a-half hours.

Health Screenings

MHA provides a variety of free health screenings throughout the community. The location of each screening is rotated throughout the Service Area. MHA provides screenings for blood pressure and cholesterol. Screenings coming soon: glucose/A1c, HIV and Hepatitis C.

Community Outreach

Our team coordinates with community partners and other entities to host events that increase access to resources and education. Additionally, we collect community feedback about health-related resources through annual needs assessments (surveys and focus groups).

Substance Use Prevention

MHA provides education to the community and resources related to opioid and substance use disorder, including care coordination for finding and enrolling in treatment programs. MHA provides free Naloxone (Narcan) and prescription lock bags to community members.

Matter of Balance

This is a free class for individuals 55 years or older. The class addresses fears associated with falling, environmental factors associated with falling and how to make positive changes, and exercises that help with strength, balance and range of motion. This is an eight-week course that meets weekly for two hours.
COVID-19 Updates:  Home visits have been put on hold temporarily in accordance with Public Health orders for COVID-19. For more information or to schedule an appointment by phone or in the office, please call (707) 412-3176 x 102 or email [email protected]  All in-person appointments require a Covid-19 symptom screening prior to entry into the office. Individuals experiencing COVID-like symptoms or a fever cannot enter the facility and will need to reschedule by phone or when a COVID-negative test result can be provided. 

This website and MHA’s programs are supported by the Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award through the Rural Health Network Development Program totaling $898,645 and through the Rural Communities Opioid Response Program totaling $1 million, over a 3-year period, with  80% funded by HRSA/HHS and 20% percentage funded by non-government source(s), such as the Arlene & Michael Rosen Foundation, Santa Rosa Memorial Hospital, the Community Foundation of Mendocino County and tax-deductible contributions from community members.
