Our Services

It is Mendonoma Health Alliance’s goal to provide person-centered programs that meet people wherever they are in their healthcare journey. Our services are available to anyone living in our Service Area. We provide home visits, one-on-one assistance in reviewing instructions from healthcare providers, help with preparing the client for doctors’ appointments, health education, as well as access to resources specific to each client’s needs. This is a 30-day program that helps connect people to resources in order to ensure a smooth transition back home after being released from the hospital.

Care Coordination

This program provides assistance and support for a wide variety of needs, such as accessing fresh food and health education, assisting clients with paperwork, working with clients to achieve health goals, etc. This program is available to clients for as long as it takes to meet their needs.

Mobile Health Clinic

Our mobile clinic runs 3-days a week serving patients with Medi-Cal who are 19 years or older, Medicare, private pay, and underinsured. We offer primary care, addiction treatment services, women’s health and urgent care related visits.

Health Screenings

Mendonoma Health Alliance provides a variety of free health screenings such as screenings for blood pressure, cholesterol, glucose/A1c, HIV and Hepatitis C. Screenings are available at our main office at 39251 CA-1 in Gualala as well as on our Mobile Clinic.

Community Outreach

Our team coordinates with community partners and other entities to host events that increase access to resources and education. Additionally, we collect community feedback about health-related resources through annual needs assessments (surveys and focus groups).

Substance Use Prevention

Mendonoma Health Alliance provides community education and resources on opioid and substance use disorder, including care coordination and a Peer Recovery Program. Addiction treatment services are also available through our Mobile Clinic. Free Naloxone (Narcan) and prescription lock bags are offered to support overdose prevention and medication safety.

Healthy Living Course

This is a free class for any person regardless of health conditions or health goals. The class teaches participants skills to create plans and achieve short-term and long-term health goals. This is a six-week course that is scheduled weekly for two-and-a-half hours.

Matter of Balance

This is a free class for individuals 60 years or older. The class addresses fears associated with falling, environmental factors associated with falling and how to make positive changes, and exercises that help with strength, balance and range of motion. This is an eight-week course that meets weekly for two hours.

This website and Mendonoma Health Alliance’s programs are partially supported by the California Department of Public Health as part of a grant award through the Substance and Addiction Prevention Branch totaling $1,281,000 over a 3-year period that began on March 1, 2024. Other funding sources that support this website include funding from Santa Rosa Memorial Hospital’s Community Benefit Program and generous donations from the community.
